I acknowledge that this Waiver and Release of Liability Form will be used by the group, 303 Concert Rides LLC and their directors, officers, representatives, and agents (the Company", and that it will govern my rights and responsibilities on the trip on which I am about to embark. I acknowledge that the Company is NOT responsible for my negligence or the negligence of other third parties, including, without limitation, negligent acts associated with consuming too much alcohol or other mind-altering substances. I acknowledge that the Company reserves the right to refuse service to me in its sole and absolute discretion if I engage in conduct detrimental to the safety of others and that I may be forced to make and pay for alternative travel arrangements in the event I am refused service by the Company. I acknowledge that the Company is not liable in the event of a mechanical breakdown while on charter and will only be responsible for making up lost time at a mutually agreed date. I acknowledge that I knowingly and willingly consent to bus transportation during COVID 19; I further acknowledge that 303 Concert Rides LLC is not liable in the event of any exposure to or result from COVID 19 while a passenger on their bus. I further acknowledge I have been verbally informed of the risk of exposure, that the CDC recommendation of social distancing of at least 6 ft is or may not be possible when aboard 303 Concert Rides LLC, and a mask will be required at boarding. I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the Company, for my death, disability, personal injury, property • damage, property theft, or actions of any kind which may hereafter occur to me unless such death, disability, damage, theft or injury is caused by the Company's gross negligence, recklessness, or intentional act. This Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
